Entry into Ukraine for authorized permanent residence
In order to qualify for permanent residence applicants are required to submit to the Consular Section of the Embassy of Ukraine in Canada three copies of the following papers:
A written request (duly signed and dated) formally declaring one's intention to apply for an authorized permanent residence in Ukraine (explaining your reasons thereof and indicating whether or not the requester will be seeking financial assistance after being lawfully admitted for permanent residence).
A Curriculum Vitae with your full name, present and permanent address, (area code) telephone number, date and place of birth, citizenship, marital status, if married - full name of the wife and husband and all legal dependents, their ages, educational background (including diplomas obtained, date of graduation for schoolchildren - last completed year of study).
A Medical Examination Certificate, including AIDS test results, signed and dated by a certified physician. This Certificate should be notarized and authenticated by county, state and federal authorities. It must be translated into Ukrainian and certified as an accurate translation by an authorized Ukrainian language translator.
A completed Questionnaire received from the Consular Section.
The above-mentioned documents should be accompanied by four (4) identical (most recent) passport - size photos of the applicant (photocopies are not acceptable).
Each adult individual must complete a QUESTIONNAIRE.
The consular fee for processing a request to be admitted into Ukraine for the purpose of permanent residence is $160 CND, payment being made in the form of a money order payable to the Embassy of Ukraine in Canada. Should one's request to enter Ukraine for lawful permanent residence be denied, the fee paid is not refundable.
