Membership year is January 1 to December 31.

Once completed, either scan and send the completed form with credit card information to ucpbator@gmail.com or if paying by cheque, please make the cheque payable to “UCPBA of Toronto” and mail with your completed form to:

Spousal: 2 x $50= $100
